        <title id="abstract-title-d1c2c401d24b4cdc83cb4a4e5c2540fd">Abstract</title>
        <p id="paragraph-f0fc67a9b052450b9d592abca4b17924"><italic id="emphasis-1">Shooka Dosha</italic> refers to a group of disorders arising from the misuse of medications intended for penile enlargement. These disorders are associated with the use of sexual stimulants or penis enlargement products that contain toxic or harmful substances, leading to pathological conditions collectively referred to as <italic id="emphasis-2">Shooka </italic><italic id="emphasis-3">Dosha</italic>. <italic id="e-b8dbfe5bb627">Acharya</italic>  <italic id="e-5c1497f40112">Sushruta</italic> identified 18 such diseases, categorizing 14 as <italic id="emphasis-6">Sadhya</italic> (curable) and the remaining 4 as <italic id="emphasis-7">Asadhya</italic> (incurable). This review examines <italic id="emphasis-8">Shooka </italic><italic id="emphasis-9">Dosha</italic> through the lens of modern medicine, correlating its conditions with the treatments available in Ayurveda, on the basis of classical texts. Currently, no clinical trials have been conducted on <italic id="emphasis-10">Shooka</italic> <italic id="emphasis-11">Dosha</italic>. This study aims to initiate exploration into this largely overlooked aspect of male reproductive health, as preserved in the traditional knowledge of Ayurveda.</p>

      <p id="paragraph-f758c139407b44d0aa4b1765934d9565">From rise of human beings there is always a concern regarding the length and strength of the penis. In a study it was concluded that in today’s era <italic id="e-17637772ba03">Hindu</italic> civilization it is said to be inappropriate to talk about intercourse whereas in ancient times <xref id="xref-f10a4a86562b45159acea3980b71792a" rid="R279114433834886" ref-type="bibr">1</xref>, <italic id="e-6a47a5c33836">Hindu</italic> mythology has gifted a complete <italic id="e-9b0291effb2f">Grantha</italic> on reproductive care and health “<italic id="e-860ac00e3816">Vatsayana Samhita</italic> <xref id="xref-ec2e2f5db7ef4191a2f28adec648b5b7" rid="R279114433834896" ref-type="bibr">2</xref>”. It describes the development of education and awareness regarding sex in ancient times <xref id="xref-a7d97c2e1dcc4873aa18bf660245aab6" rid="R279114433834874" ref-type="bibr">3</xref>. Ayurveda is rich in medical and life living heritage where a major place is assigned to <italic id="e-11e406f58825">Brahamcharya</italic> (rules for sexual activity) and <italic id="e-899ce4dd1be0">Vajikarna</italic> (sexual power enhancement) <xref id="xref-74276642f015495f9ede979b30e6751a" rid="R279114433834835" ref-type="bibr">4</xref>. In westernized world it a taken a wrong aspect regarding necessity of enlarged penile length <xref id="xref-2696fb7c752b42c59c157c08727ec773" rid="R279114433834885" ref-type="bibr">5</xref>, earlier also males with the larger genital organs were regarded in superior strata than rest <xref id="xref-cfd5fe78edff4bf2ae39a186441c5232" rid="R279114433834882" ref-type="bibr">6</xref>.</p>
      <p id="paragraph-bfaf92ced4e440059c43f7f6658a93e7"><italic id="e-e66c9505fd01">Shooka Dosha</italic> is term given to cluster of disorders arising from the improper use of medications intended for penile enlargement. The term <italic id="e-4880d4113513">Shooka</italic> refers to sexual stimulants or penis-enhancing substances containing toxic or harmful compounds, which result in pathological﻿ conditions which may occur as an individual disorder or multiple at a time, identified as <italic id="e-073afd6a1e83">Shooka Dosha</italic> <xref rid="R279114433834851" ref-type="bibr">7</xref>, <xref rid="R279114433834859" ref-type="bibr">8</xref>. In <italic id="e-8b8d4e490893">Madhava Nidana</italic> commentary <italic id="e-cc31e71807d6">Madhukosha, Shooka</italic> is referred to a poisonous insect that is produced from extract or waste products in water. Drugs made from these insects are also referred as <italic id="e-df813633b488">Shooka</italic> which are applied to increase the length of the penis <xref id="xref-0046b39f4d4a4dbfac43e1940e20ee0c" rid="R279114433834870" ref-type="bibr">9</xref> <italic id="e-cf795bb73c5d">Acharya Sushruta</italic> <xref id="xref-af37ba357fd74afeaa8805cc2f9a779c" rid="R279114433834877" ref-type="bibr">10</xref>, <italic id="e-3896382a7ae5">Acharya Yogratnakar</italic> <xref id="xref-8356a9b4304e44f69c519acbc1efa5fa" rid="R279114433834828" ref-type="bibr">11</xref>, <italic id="e-5ca5322cf6dd">Bhaisajya Ratnavali</italic> <xref id="xref-7f6389c7205346b798700245a1cc4227" rid="R279114433834858" ref-type="bibr">12</xref>, <italic id="e-abd5dc27a369">Madhavkar and Bhavaprkasha</italic> <xref id="xref-83ea03279eef47c480261a1de4ec5ac9" rid="R279114433834857" ref-type="bibr">13</xref> had described the 18 diseases with their <italic id="e-3c8a8f5908a9">Lakshana</italic>, <italic id="e-8a943592e11a">Sadhyata-Asadhyata</italic> and <italic id="e-f425f0c10a08">Chikitsa</italic>. Among these 18, <italic id="e-d88b40d80280">Acharya Sushruta</italic> considered 14 as <italic id="e-8c752f2ae477">Sadhya</italic> (curable) and rest 4 as <italic id="e-86d618095148">Asadhya</italic> (incurable).</p>
      <sec>
        <title id="t-596eb2fd7ce5">1.1 <bold id="strong-4846faa1f9d64b7cb199390a71a40446">Aim and Objective</bold></title>
        <p id="paragraph-7c7520a9c7b54d2698cfb9ca21ad54d5">To study the concept of <italic id="e-1e06cd6ae655">Shooka Dosha</italic> described in Ayurveda texts and its relevance in the modern era.</p>
      </sec>
    </sec>
    <sec>
      <title id="title-c75f86355dde4199a016462069f5fa68">2 Material and Methodology</title>
      <p id="paragraph-384c792d63484c4da175c78b9db9fff3">Collection of the data was done using the original text, online resource including journals of Pub Med, Web of Science, and Ayush Research Portal were searched with term “<italic id="e-734f1b86f016">Shooka Dosha</italic>”. The gathered literature was subsequently reviewed and thoroughly analysed to ensure a comprehensive understanding of the <italic id="e-6b125d289398">Shooka</italic> <italic id="e-24399c433e67">Dosha</italic>.</p>
    </sec>
    <sec>
      <title id="title-c287b61a25a94939bb4472bcbec8cb01">3 Discussion</title>
      <p id="paragraph-61ee0b86c53a421689109485004508e0">The use of <italic id="e-b5d9fa888077">Shooka</italic> (remedies prepared with poisonous drugs for local application, with intention to increase the size or length of the penis) leads to <italic id="e-bb46bae45e4b">Shookajnya Roga</italic> mentioned in Ayurveda text. <italic id="e-4bb6a9c59246">Acharya Sushruta</italic> mentioned 18 <italic id="e-f12e1974d572">Shooka Roga</italic>. Symptoms are described individually as indicated in <xref id="x-ee5c0c36c48a" rid="table-wrap-01bc40bf5ded42d584b4f35613927781" ref-type="table">Table 1</xref>.</p>

        <p id="paragraph-4e22e2a9f8c34dc287b9fa8b0f9cf0b1"><italic id="e-efc2f7be5531">Acharya Sushruta</italic> had mentioned 18 types of <italic id="e-2ac823b66c53">Shooka Dosha</italic> out of which 14 are <italic id="e-8b6ed6e87754">Sadhya </italic>(curable) and 4 are considered as <italic id="e-b750db49bb66">Ashadya Shooka Dosha</italic> (incurable) [<xref id="x-479037cafe54" rid="table-wrap-3e6d229647d24f818d66d88925acec7f" ref-type="table">Table 2</xref>].</p>
        <sec>
          <title id="t-82a6023ba608">
            <bold id="strong-3f81cecee6ad406f84206f6adafd8aef">Treatment Measure </bold>
          </title>
          <p id="paragraph-7d7d3b3683da45cea1ca95773d1e0f95">An identified curable <italic id="e-48089b4e5b44">Shooka</italic> has a detailed line of treatment mentioned in Ayurveda. Both the generalized and specialized measures are indicated in text. Generalized concepts are applicable to all entities while specialized are for an individual entity.</p>
        </sec>
        <sec>
          <title id="t-10a426b3eb0c">
            <bold id="strong-1ecfb20ccad6425dbd6e022d78d3a5a5">General measures </bold>
          </title>

              <p>Acharya <italic id="e-636e11618fb2">Sushruta, Vagbhatta, Yogratnakara, Bhaisajya Ratnavali and Bhavaprkasha</italic>, adapted similar principles to manage <italic id="e-b6faf3b57661">Shooka Dosha</italic>. Appropriate use of <italic id="e-1abb7c33e623">Kashaya</italic> (decoction), <italic id="e-972dbd22184f">Kalka</italic> (paste),<italic id="e-598a18c42e83"> Churan</italic> (powder), <italic id="e-8c671d63a053">Taila </italic> (oil), <italic id="e-665a1f130aec">Ghrita</italic> (clarified butter)<italic id="e-023c96b62ca2">, Rasakriya</italic> (linctuses), and <italic id="e-79bb49415228">Shodhana‑Ropana</italic> (cleansing and healing). Apart from this intake of <italic id="e-4f67e035e4ad">Ghrita</italic> followed by <italic id="e-1d7fcc0591f4">Virechana</italic> (purgation), <italic id="e-6ef73314a732">Raktamokshana</italic> (bloodletting), and other treatments measures are similarly adapted for wound management and <italic id="emphasis-13">Visha Chikitsa </italic> (treatment measures that are taken for poisonous conditions). <italic id="emphasis-14">Bhav Prakash</italic> <xref id="xref-89686bc1212940768fee5014caf03b89" rid="R279114433834844" ref-type="bibr">14</xref> indicated application of <italic id="e-d8fd03a90146">Triphala Kwath </italic> (decoction of<italic id="e-9533e790ff0a"> Amalaki - </italic><italic id="e-2ff641f8df17">Emblica officinalis</italic><italic id="e-835ad129ff3e">,</italic><italic id="e-3879dca415bd"> Vibhitaka<italic id="e-12e914282dbb">- </italic></italic><italic id="e-1febb3959492">Terminalia bellerica</italic> and<italic id="e-79bdddb0301b"> Haritaki- </italic><italic id="e-f999bdeb609d">Terminalia chebula</italic>) treated with <italic id="e-2cae9acae165">Gugglu (Commiphora wightii)</italic>, Irrigation with cold milk. The use of specific oil and paste like <italic id="e-a018a92d4e14">Darvi Taila</italic> and <italic id="e-1f950d8494b6">Medhra Roga Nivaryet Rasanjana Lepa</italic> (<italic id="e-f365c13fb2b5">Sarva Anagroga</italic>) indicated by <italic id="e-dc409a2ea629">Yogratnakara</italic> <xref id="xref-a62517e1b21d4bd4b94ca87b043ae126" rid="R279114433834839" ref-type="bibr">15</xref>. Intake of <italic id="e-2e10c9330937">Ghee</italic> along with <italic id="e-8f37754027b5">Pitta</italic> suppressing drugs, diet and procedures like <italic id="e-f294516af015">Virechana</italic>, bloodletting <xref id="xref-9b718215e3c54c13b3c25b1416c1a5bc" rid="R279114433834866" ref-type="bibr">16</xref>, application of paste, <italic id="e-c21eca89df0d">Rasa Aushadh </italic> (herbo- mineral drugs) and measures adapted for wound management like <italic id="e-8c0ffc3c1ffe">Shodhana</italic>, <italic id="e-e522d77ad3f9">Ropana</italic>, <italic id="e-6404c71a1cc2">Patana </italic> (Incision of lesion).</p>

Similar line of treatment mentioned by <italic id="e-f2787ef472c9">Rasakamdhenu</italic> <xref id="xref-e1b074f718a549849f114a8917aabc49" rid="R279114433834843" ref-type="bibr">20</xref>. <italic id="e-df6c5fa2d735">Vangasena</italic> <xref id="xref-833b18dba2d9419abff5c30463b261b4" rid="R279114433834836" ref-type="bibr">21</xref>, <italic id="e-6a31abb728a5">Yogratnakara, Bhaisajyaratnavali</italic> mentioned the line of treatment like <italic id="e-bce0cbd84a14">Vagbhatta</italic>. <italic id="e-17bc71f8cb5a">Vasavraj</italic> <xref id="xref-9b3c0989e3de4cfeb243962fd036fe4f" rid="R279114433834881" ref-type="bibr">22</xref> <italic id="e-16777014e3cf">Vagbhatta</italic> indicated bloodletting, Treatment similar to <italic id="e-96d3ba249f5f">Kaphaja Granthi</italic> (swelling occurred due to predominance of <italic id="e-51c10c2a41d1">Kapha Dosha</italic>) along with irrigation by <italic id="e-26f76a2b34a9">Tinduk (Diospyros tomentosa Roxb.), Triphala </italic> and <italic id="e-cba26027a861">Lodhra (Symplocos racemosa Roxb.)</italic> in the treatment of <italic id="e-6081403c5d55">Ashthila</italic>. </p>

              <p><italic id="e-1370cb94b1c8">Grathitha</italic>- Regular <italic id="e-f798e9c32da2">Swedana</italic> (Sudation therapy) with use of <italic id="e-e14805b61b58">Snigdha </italic>(slimy or unctuous) and <italic id="e-89d5d7fb3b5d">Koshana</italic> (lukewarm) poultice <xref id="xref-6ba3871b0a99477e8feb3057ad535962" rid="R279114433834891" ref-type="bibr">23</xref>. Similar line of treatment adapted by <italic id="e-1a1248802946">Vangasena, </italic><italic id="e-6c911b8e70e9">Rasakamdhenu </italic>and<italic id="e-f584ebc6028e"> Bhaisajyaratnavali</italic>. <italic id="e-84d54a708c9a">Yogratnakara</italic> also mentioned similar to <italic id="e-10eed7151611">Shusruta</italic>, additionally indicated application of poultice mentioned in <italic id="e-1bba4395b2d9">Vrana Chikitsa</italic> (management of wound) <xref id="xref-afeff8d63d8e48bca92f8cc06ee63df8" rid="R279114433834840" ref-type="bibr">24</xref>. <italic id="e-09a2322a6651">Vasavraj</italic> indicated the same, repeated use of sudation and application of poultice treated with lukewarm oil. </p>

              <p><italic id="e-f5543d8bc5fe">Kumbhika</italic> – Suppurated <italic id="e-6f1ed1ac5c6f">Kumbhika</italic> is excised with instrument and after cleaning wound, healing is promoted by <italic id="e-f0754d4fdbb3">Triphala, Lodhra, Tinduka</italic>, and <italic id="e-7615b28eb5c0">Amra (</italic><italic id="e-7e33edf64c2d">Mangifera indica</italic><italic id="e-c20644e04d07">)</italic> <xref id="xref-4fe262e14a704514bcd37dfcc154c45a" rid="R279114433834863" ref-type="bibr">25</xref>. Similar concept adapted by <italic id="e-403642967b52">Rasakamdhenu</italic> while <italic id="e-907f3a998746">Vangasena, </italic><italic id="e-34ef71b20d06">Vasavrajiyama, Yogratnakara, Bhaisajyaratnavali </italic>follow the <italic id="e-08ae902ab489">Vagbhatta</italic>.</p>

              <p><italic id="e-51631855a855">Alaji</italic> – Bloodletting by Leech therapy followed by irrigation by decoction of astringent drugs, for wound healing use of same drugs decoction and paste cooked oil <xref id="xref-17385c94d72744f88b74346a20b657c8" rid="R279114433834831" ref-type="bibr">26</xref>. Similar concept adapted by <italic id="e-ab3d2faca3cd">Rasakamdhenu </italic> while <italic id="e-fe8bc425aebb">Vangashena, Vasavraj, Yogratnakara </italic> and<italic id="e-e26401e85c14"> Vaghabhat </italic> also follow the same line of treatment. </p>

      <p id="p-3b57f21700f7"><italic id="e-92447119e338">Shooka Dosha</italic> is an important concept but yet no conceptual or clinical work carried out. More research work is needed to understand its importance. Sincere efforts were done by authors to compile the data and made available related to <italic id="e-a1a6762ef9df">Shooka Dosha</italic> from Ayurveda literature. The traced facts were collected systematically and then analyzed. In the first observation, these diseases are mostly seen in society and can be easily correlated with modern existing penile disorders. The searched aetiology, clinical features, and their treatment modalities can be utilized. Clinical trials can be run at the post-graduation dissertation and doctorate work to make it more validate. It is also suggested to make evidence‑based clinical correlations of the <italic id="e-40afa0544cbe">Shooka Dosha</italic> of Ayurveda with modern science. The research‑based data publications are also solicited to put the subjects on scientific platform. The evaluation of efficacy of mentioned drugs in <italic id="e-f43722250353">Shooka Dosha</italic> for the antibacterial and wound healing properties needs to explore regarding the treatment of genital disorders. </p>
      <p id="p-2fb35ee9ae55">The exploration of <italic id="e-9097a8af3527">Shooka Dosha</italic> through classical Ayurvedic texts reveals a nuanced understanding of disorders associated with inappropriate use of penile enlargement medications. Through a detailed examination of classical Ayurvedic literature, particularly the work of <italic id="e-628da3d2525c">Acharya</italic> <italic id="e-e7e017667407">Sushruta</italic>, the research correlates these ancient conditions with analogous symptoms recognized in modern medicine. By categorizing the 18 distinct disorders into curable and incurable forms, as outlined by <italic id="e-96ef33092b83">Sushruta</italic>, the study highlights both the pathological consequences of toxic substances used in penile enlargement and the extensive treatment protocols in Ayurveda for each condition. These conditions, as elaborated by <italic id="e-65628bf9d7e3">Acharya</italic> <italic id="e-df030d0aef09">Sushruta</italic>, present a unique framework that could complement modern approaches in understanding and addressing penile health disorders.</p>
      <sec>
        <title id="t-6e8b549efa7b"/>
        <p id="p-9cc8c9a88cea">Through comprehensive literature review and correlation with modern medical conditions, this study highlights the potential of Ayurveda in offering both preventive and therapeutic insights into <italic id="e-2415d4a66a89">Shooka</italic> <italic id="emphasis-12">Dosha</italic>. The principles and treatments suggested, including bloodletting, use of herbal formulations, and detoxification techniques, offer a holistic alternative for curable cases and a respectful approach for incurable cases. This research intends to lay the groundwork for further clinical studies to substantiate Ayurveda’s efficacy in treating these disorders, thereby bridging ancient wisdom with contemporary health challenges.</p>
